First Visit

We want your first vist to Golden Triangle Physical Therapy to be comfortable one.

You will need the following items:

  • Your medical prescription
  • Your insurance card
  • Any x-ray / MRI reports which you are in possession of
  • Appropriate clothing such as T-shirts, tank tops, and gym shorts
  • New patient forms
  • Medicare forms if you are a Medicare patient
